Open or Switch German Statutory Health Insurance (GKV): Step-by-Step Guide

Key Information Upfront: Start Date and Gap-Free Coverage

For Switchers:

You can switch your health insurer if you’ve been insured with your current provider for at least 12 months. The notice period is two months to the end of the month (e.g., apply by March 31 to start with the new insurer on June 1). Simply sign up with a new provider, they usually handle the cancellation automatically.

Special Right to Terminate Early:

If your current insurer raises their additional contribution rate, you can terminate before the 12-month binding period ends – with a notice period of two months to the end of the month. Your new insurer handles the cancellation for you.

Example:
Your insurer raises rates effective July 1.

  • Apply by May 31 → Switch on August 1

  • Apply by June 30 → Switch on September 1

Tip: Always submit your application two full months before your desired switch date!

For New Arrivals:

Your statutory coverage typically starts with your employment contract or semester (e.g., April 1 or October 1). Important for visas: GKV does not start automatically upon entry. For the days between arrival and work/semester start, you usually need private transitional insurance („Incoming Insurance”), such as HanseMerkur.

Before You Start: Documents to Prepare

To complete the online application without interruptions, have these details ready:

  • For switchers: Name of your current German health insurer
  • Social security number: If you’ve worked in Germany before (found on your payslip)
  • Employer / University: Name, address, and start date of your current or new contract/studies
  • Contact details: Email, German mailing address (or where you’ll soon receive mail)
  • Passport photo: Digital photo for your electronic health card (eGK)

Tip: If you’re about to move, use an address where you can reliably access the mailbox. That’s where your health card will be sent after coverage begins.

Step-by-Step: How to Open Your GKV

Step one: Status and Personal Details

In the online form, first select your status: Employee, Apprentice, Student, Self-employed/Freelancer, or Unemployed/Family coverage. Enter your personal details exactly as they appear on your passport or ID.

Address Tip: It’s ok to provide your home adress outside Germany, but take care to change the adress in your account once you have found an accomocation here.

Step two: Details About Your Situationn

The form adapts based on your selection. Key points to note:

Employees and Apprentices
Enter your employer (name, address) and gross salary. The insurer contacts your employer automatically later – just inform them briefly which provider you’ve chosen.​

Students
Membership is tied to the semester (start usually April 1 or October 1). Enter your university and semester start date.

Self-Employed/Freelancers
Provide your start date for self-employment and estimated monthly income. Your bank details (IBAN) are mandatory since you pay contributions yourself. Contributions are debited automatically, with a final adjustment made following your tax return. Consequently, you may receive a reimbursement for overpayments or be required to make an additional payment if there was a shortfall.

Step three: (Only for Switchers) Previous Coverage

Enter your current German insurer (name, possibly member number). Once submitted, the new insurer electronically coordinates with the old one – no paper cancellation needed.

Step four: Family Members and Photo

Family Coverage: Spouses and children (up to 25) are covered free until their own income reaches 565 Euro or 603 Euro for minijobs. Include their details if applicable.
Photo Upload: Upload your/their digital passport photo(s) directly – speeds up electronic health card (eGK) production.

What Happens After Submission?

  • Instant Confirmation: You will receive a confirmation of receipt via email immediately
  • Provisional Certificate: Your (provisional) membership certificate will arrive as a PDF within 24–48 hours – suitable for visa applications, employers, or university enrollment
  • Provider Switch: If switching, your new insurance provider will automatically notify your old one
  • Health Card (eGK): Your electronic health card will be sent by mail 1–2 weeks after your coverage officially begins
